The oil & gas industry has always focused on health, safety and environment (HSE).  Despite this focus, the industry is not without incident.  After the Piper Alpha explosion and fire in the North Sea in July 1988, an industry consortium of five major energy companies pooled their resources to ensure that such a tragedy would never reoccur.
